If you are unable to make your appointment please cancel within 4 hours of appointment time otherwise a cancellation charge of £20 may be charged

If you have any of the following health issues – you may need a referral from your GP 

• Anyone currently receiving treatment for cancer, any serious lung condition, anyone recently post-operative

• Experiencing severe post Covid-19 circulatory complications – DVT, micro-embolisms, CVA (Cerebrovascular accident or stroke) or PE (Pulmonary embolism –  a blocked blood vessel in your lungs)

• Aged 70 years or older – if you suffer with ill health

• Pregnancy 

• Mild heart & respiratory conditions and suppressed immune systems

• Diabetes

• BMI over 39

Please be aware that I may have to cancel your appointment if I am unwell. Likewise I will give you at least 4 hours notice beforehand.

I will use my professional judgement for each client to decide whether to give therapy or not. 

Please will you fill in this consultation & declaration form prior to coming for your massage. Let me know if this will cause any problems as other arrangements can be made.
